District 12 Parent Voice • Independent & parent-led
Privacy
Understand how your information is used before sharing your experience.
Effective October 14, 2026. This policy covers submissions to District 12 Parent Voice.
What you choose to share
We collect survey ratings, written responses, publication permission, and optional school or grade context through Squarespace forms. Names and emails are optional. Leave both blank and avoid identifying details in your writing to submit without providing your identity.
What “anonymous” means here
We do not require your name or email. Complete technical anonymity is not promised: Squarespace, its form services, and spam-protection providers may process IP addresses, browser details, cookies, and security signals. Do not submit highly sensitive information or private student records.
Private review and publication
Site administrators review submissions privately. Contact details are never published. Combined results may include your ratings. Written responses are published only under the permission you select and after moderation. Named publication without a provided name is treated as anonymous permission; we do not infer identities.
Storage and access
Forms are hosted by Squarespace. Optional-email submissions remain in the form block for 30 days. Administrators must export records privately before that window expires to retain them for review. Administrators may use private form records, exports, and notification emails to review responses. Submission records are not public. Information is not sold or used to build marketing lists. Private exports receive the same access protections.
Retention and requests
Identifiable submission records are reviewed for deletion within 12 months after collection, unless needed for a pending correction or request. Published aggregate summaries may remain. To request a correction or removal, use Parent Questions, choose “Keep my question private,” and include enough detail to locate your entry. Anonymous entries may be impossible to locate without those details. External copies of published content can be outside our control.
For parents and guardians
This site is for adult parents and guardians. Do not submit children’s names, student IDs, diagnoses, schedules, or other identifying information. Contact the district directly for individual student matters or official complaints.
