This form is open to anyone affected by this site. You do not have to be a parent, an adult survey participant, or the original submitter to report a concern. Every request stays private and is reviewed by Crystal Larson. It will not appear in survey results or public quotes.
Include a page link, approximate date, and a brief excerpt that helps locate the issue. Do not include student names, identity documents, medical or education records, or sensitive details. If you are under 18, a trusted adult can help you describe the concern without identifying you.
Name and email are optional. Include an email if you want a reply; it will not be published. The owner aims to acknowledge requests within seven days and resolve them within 30 days, subject to applicable deadlines. Anonymous records may be difficult to locate or verify.
This form is not continuously monitored. For emergencies or official student matters, use the appropriate emergency or district channels.
